My grandson has been living with me,my daughter wants to write a letter and have it notarized.To give me permission to seek health care for my grandson if he was to get sick.He is a two year old,I have been providing all his needs. She needs to know what she should write,and I need to know if she does this will I be able to seek care for him.

A guardianship document can be prepared and notarized, giving you guardianship, or you can file for custody and she can agree to that without going to court. You may want to check with your health insurance to verify what they require.
