Guardian Form

Were can i get the Guardian Form or must it be written by a lawyer?

When i went to the court building i was told that there is a special form that a lawyer had to give me.

Why is this so when me and the birth mother was there together?

First off, neither the clerk or I have any idea what a "Guardian form" is.

Guardianships are for people who need some one to manage their affairs due to senility or other disabling condition. Sometimes guardianship of a child maybe given in the event of the child becoming an orphan.

Tip when asking for help state what you want to happen don't get hung up on the forms required. What you want may not even require the form you're asking about.
