Legal Question in Family Law in Pennsylvania

Me and my kid's father do not have a court appointed custody agreement; and until now we've never needed one. He now has my twin boys and refuses to give them back, is this legal? Their father has mental issues and is "supposed" to be on medication but he refuses to take them, so as a result he is not always stable. How can I get them back as soon as possible.
