Maiden Name

Hi, when I got divorced a couple years ago, I did not change my name back to my maiden name. I now wish to do so. I was able to have my name changed on my license, registration, insurance cards etc. THe only thing it is not changed on is my social security card.

My divorce papers did not state i could change my name back to my maiden name since i was not planning to do so at the time. My lawyer has since left her practice and is living in another state.

I was told I need a court order or my divorce papers to state i can change back to my maiden name.

Do you know any other way I can do this. I am now disabled and do not have enough income to pay a lawyer or to go to court. Is there a way to amend my divorce papers since I have been divorced for 4 years now?

Legal aid may be able to help. Otherwise the County Court's law library should have a book of commonly filed motions. The name change after divorce should be in it.
