power of attorney for incapable parent

How do I go about obtaining a power of attorney for my mother who can not sign her name, can not speak well and has some dementia ? Is it possible to do?

You will have to proceed with a guardianship for this matter because it sounds as though your mother is no longer competent to sign a Power of Attorney appointing you as her agent. This is relatively simple unless someone contests your appointment as guardianship. You will need an attorney to help you with this.
