How do you sign over the rights to your child?

My daughters dad wants to sign over his rights, and I would like to have this done as soon as possible, how to I go about doing this?

When you say sign over his rights are you asking about an adoption or just giving up custody rights? Either way he will need to fill out an affadavit making clear his intentions and granting his consent.
