The effect of not attending depends on several things. First, if it is a criminal case, then you had better find a way to get there because the party that had the subpoena served is probably within a few minutes of the location of the Sheriff's or U.S. Marshall's Office and they will enforce it.
In a civil case if the subpoena came by mail or was not accompanied by
a witness fee and mileage, then it is probably not enforceable although some Judges may take a different view. It is best not to take a chance and simply honor the subpoena.
If it is a subpoena for an administrative proceeding you may want to call the Judge who issued the subpoena and explain your problem and ask if you can participate by telephone.
Also, right now, video availability is very reasonable and more and more of the courts are accepting the use of such devices. If you were sent the subpoena by an attorney in a civil case then you may want to call him to see if alternatives are available.

If you don't show up then expect a visit from law enforcement to escort you to court. Health reasons don't cut it unless you're in the hospital. Even then the court may come to you.
I have served a lot of friendly subpoenas but I believe they are the exception.
