Can I refuse to answer a question in a
child custody case (not my child)?

As a witness probably not. Child custody is a civil matter so you can't "plead the 5th". You do not say if you are a person who might assert privilege. Nor do you have the right to object to a question. That's the lawyer's job.
If you do not answer you risk being in contempt.
