Legal Question in Family Law in Pennsylvania

My wife has a court ordered custody agreement from 2008 , giving her custody of her child and then in 2009 signed an informal agreement at a notary giving her ex husband custody,,, does the original court ordered agreement trump the notary agreement and does she still in fact have legal custody of her daughter? There are issues of P.F.A's dating from 2006 and the ex husband has been implying threats of violence with his new wife and my wifes' daughter,which is why we are inquiring into which document is indeed the legal and binding custody agreement in Pennsylvania.

You should consider a petition for modification of custody because of the change in circumstances, namely possible increased danger to the child etc.
