What form or format do you use to file a complaint to an appeal by the defendant who lost by default?

There are some forms found on the judicial system's website:
http://www.courts.state.pa.us/Forms/Default.htm.
If your type of action does not have a form, then there are no forms and you will have to draft your own complaint. There is no special form for an appeal of a small claims action. Once appealed, the case begins anew.
