Liability waiver Nursing care

I need a legal form to have a care nurse to sign for private duty nursing so the patient or POA is not liable if nurse hurts herself while caring for patient.

I'm not sure if such a form would be valid. It sounds to me as if it may violate workers' compensation law in Pennsylvania.
