medical directive/dying will
I have decided to stop all treatment of a chronic disease that I have suffered with for the last 18 years. How do I put this in a legal form so that it cannot be challenged or changed by my family, doctors, or some judge.

You need an attorney.
Though there are various steps you can take to control your care, if you are truly concerned about interference, you will need the assistance of an attorney both to ensure you take the appropriate procedures and to later verify your competency if your choice is questioned. Finally, the independent advice of an attorney is invaluable when making emotional decisions that may go against the wishes of people close to you.
Given your compelling circumstances, you may be able to find an attorney willing to assist you (at least to some extent) without charge, perhaps at a public interest organization, or perhaps pro bono. A public interest organization that specializes in Elder Law would be a good place to contact first.
