Driving Record Disclosure

Good morning, Is it legal in the state of PA to require a salesperson to sign a Driving Record disclorure form for the insurance underwriter? I recieve no gas, insurance, maintenance, or mileage re-imbursements and use my own vehicle. This is being asked of all employees, sales, internal departments, etc.

Employment is generally at will and if the job requires it, your only recourse is to find another job.
