Legal Question in Employment Law in Pennsylvania

i was hurt at work, i was told to sign a paper stating i was not hurt at work or risk termination. being a parent of 5 and no other job oppurtunity i did. two weeks later i get laid off.the conversation included such things as- employer repeatedly telling me he will allow me to collect unemployment but i am not to tell them i was hurt at all.

this same employer ,about a year ago, decided all employees will take one full work day "off the books" or we wouldnt be able to work that day.

legally what are my rights as far as unemployment? should i pursue a lawsuit?
