Can I prevent an employer from releasing personal information such as my home address and home telephone number to other employees without my knowledge or permission?

Probably not. But there is no harm in asking them to hold your informaton private.
Privacy rights are a very hot topic in the area of employment law. But, basic information like your address and telephone number is not specifically protected.
Does your employer have a reasonable basis for requiring this information from you? I don't think that anyone would argue that it does.
The extent to which your employer can give out that information is not as clear. If you can argue that you advised your employer you wanted the information to be held confidential, your employer disclosed the information in a context where it did not have a reasonable business interest to do so, and you were actually harmed by the disclosure, you might be able to sue for intrusion of your privacy rights.
On the other hand, if I were defending the claim, I would show how easy it is to determine your address and telephone number (even if it is unlisted) through other means. I would then argue that the information you are trying to protect is readily available in this day and age and that you, therefore, did not have a reasonable expectation that such information was private.
If your employer received compensation for releasing that information, the potential that you could maintain a claim would increase.
