back surgery
i had my back operated on over 10 years ago to remove a herniated disc, now the same disc is herniated again. how can that happen?

It can happen absent any negligence. The performance of surgery for removal of a herniated disc does not mean that the entire disc has necessarily been removed. The length of time which preceded your recurrence sugggests loose fragments were not left behind. Discuss this further with your present treating physician.
