According to the Pennsylvania code 28 563.11, the patient is entitled to access to a copy of the medical records.

This does mean ALL of the medical records.

On occasion, I have received less than all of the medical records. This is usually straightened out with a phone call to the medical records department.

If you're seeking records because you believe you may have a claim for medical malpractice, you should contact an attorney as soon as possible, regardless of whether you have obtained your records.

Fed law requires that on request you receive any electronically stored health info within 30 days. This law also prevents you being charged more than the actual cost of such records.

I agree you should consult a lawyer if your reason for obtaining records arises out of injuries suffered as a result of questionable medical care.
