Legal Question in Personal Injury in Pennsylvania

on a power of attorney form from an auto accident to to my car over to the insurance co they spelled my last name wrong it has to be notarized is this ok or ask for a new one

Sure you can ask for a new one. Alternatively, BEFORE you have it notarized, you can cross out the misspelling, initial it, and replace it with the proper spelling.
