I had a slip and fall at work and scrapped my knee on ice in the parking lot when I got off of work. Now my part time job wants me to fill out paperwork that gives the right to go into my medical records and it will stay on file for 2 years do I have to give them the right get all my medical records, it even states if I have mental health or HIV, I'm not suing is it my fault they didn't clean the parking lot?

I do not know why they are requesting that, unless you filed a workers compensation claim. I would have to review the form. Since you said you were not working at the time, you may have a claim for personal injuries.
Injuries in the parking lot may entitle you to additional benefits through workers compensation. If the lot is owned buy someone other than your employer, you may be able to sue the property owner if they were negligent. Your part-time employer has no right to your medical records based upon the information that you provided. However, if the injury is preventing you from working both jobs, you may need to provide them with proof of your inability to work. You should get representation. I handle workers compensation cases and third party negligence actions throughout the state. Please call me for a free consultation.
You may have a workers comp claim and also a personal injury claim--it's hard to say from yoru facts. Feel free to give our offices a call: 267.639.3105
