I slipped on ice at work during my 15 min. break. I hurt my back, knees and pulled the muscles in my shoulders. I did report it to my work. Would it be unreasonable for me to ask to be paid for the hours I have lost and also ask to start with a clean work record?

You probably have a work comp injury. Work comp pays for your medical bills and for all wage loss after the first 5 days.
