How can I sue someone for defamation of character in Pennsylvania?

There are no "forms" for such a lawsuit. You prepare one from scratch, or, more wisely, hire an attorney to prepare one. You really do not want to represent yourself in a defamation case.

There is no form. Go to law school for 3 years. Practice law for 25 or more years. Craft a complaint. Pay your filing fee and file it. Serve it. Conduct 15 depositions. Prove damages. Go to trial. Win. Win on appeal. (and most importantly:) collect the money the jury awarded you.
