When I got divorced the house was signed over to my ex but he kept my name on the morgage until recently did I have to sign any papers when he refinanced the house to remove my name.

Unless the bank agreed, your name couldn't be removed from the mortgage.
If the house is now in only his name and he refinanced, only his name should be on the mortgage.
You may go to the County office that records deeds to see what the current deed and the new mortgage say. If your name is on either of those documents, you will have to have them corrected. Have a lawyer help you for that.
