Reporting an unsafe driver

my wife has a cousin whose husband had a brain annurism and subsequently a portion of his brain removed. They had lived in Florida and that state refused to renew his driver's license. PA on the other hand renewed it!! In PA you just show up and have your picture taken. He has the mind of perhaps a 9 year old. He has done some VERY dangerous things while behind the wheel.

We need some advice as to how to get his driving priviledges revoked.

You asked about getting the license revoked of someone who has had a portion their brain removed.

At least he has an excuse for driving like a half-brained twit. How do we explain the rest of the drivers in our state?

Look on the commission's web site and it gives explicit directions as to how to have an inquiry launched. You can also discuss the issue with the person's physician as s/he has a duty to inform the commission of conditions that will likely have an effect on driving ability.
