if a traffic ticket (form) is a legal document, and it is written incorrectly, is it a binding legal document?

No, a traffic ticket is not a legally binding document meaning it is not an agreement between you and the officer.
However, there are a few occasions when an error in the charging document can lead to its unenforceability. Theses are not many and most cases, the police officers is permitted to amend the citation at the hearing or withdraw it altogether and file a new one. Then, your case depends upon whether you have suffered any prejudice from these actions and/or if the actions by the cop were done intentionally to harass you.
