Collection of Property of Deceased Daughter
My deceased daughter has a bank account with a balance under $100. She did not have a will, a spouse, or any children. She did not have any net assets after funeral expenses. I've been asked by the bank to submit a Affidavit for Collection of Personal Property to settle the account, and a Letter of Direction indicating the address to where a check should be mailed. Where do I find these forms so I can complete them, have them notarized, and submit them to the bank. The bank is located in Minnesota.

Just ask the bank to provide you with their forms.
