My father wants to revise his will, could you tell me what form I need to get?

It isn't the form but the requirements for a valid will. Which in Pennsylvania isn't much.
If you expect to be a beneficiary of the will you can have no hand in drafting the will. Or it could be challenged because you had undue influence.
Its up to your father to take care of the matter.

In theory, you could draft the amendment or 'codicil' as it is called, on a napkin--PA has very relaxed rules for the format. There are requirements that must be followed, however, there is no one approved form. BUT...be very careful! While there are some things that the layperson can do for himself/herself, drafting wills and codicils is one of those things that you should consider seeking professional advice on if there is any appreciable wealth involved in the estate in question, or if the testator's wishes must be carried out without fail--all lawyers have seen instances where poor drafting has created unintended results.

You should not be preparing a will for your father. He should consult with an attorney of his own, and in complete privacy - that means with no one else present, not even you.
Sorry to be so brusque, but it is better for you and for him that way.
