Power of Attorney

My wife and I have designated an individual to have the power of attorney if we're incapacitated - what do we do if we want to change that individual? Can we simply change the name on the form and initial the change, or do we have to have a new power of attorney form worked up?

Best practice would be to draft new power of attorney forms including language revoking any prior power of attorney.
