How should you word the beneficiary when the beneficiary is a special needs trust identified in a will?

Do not do a special needs trust on your own and do not do a will on your own leaving assets to a special needs trust. Pay a lawyer to draft it for you.

I am not sure what you are doing. Are you trying to fill out a beneficiary form for life insurance or something like that (like an IRA/401(k)?) If so, you might say something like "I direct that my assets be left to the beneficiary, the [name of trustee], the trustee of my Special Needs Trust created for my [name of person] dated month/day/year."
