my wife and me the father of the child decided to change the surname of the child and also to put my name as a father but my wife and me live together for four years,becoming five and the problem were not married yet we dont know what to do and the requirements? the only idea is we can process this at caloocan city main nso,

Just proceed with the local civil registrar (Caloocan not NSO) where the birth was recorded and they will furnish you the requirements.
