we will be getting married in a Christian Church this november and one of the requirements that is being asked us to submit is an affidavit of cohabitation can you tell me about this affidavit? thanks.

This is only required if you lived together as husband and wife without the benefit of marriage for at least 5 years as an exception to the marriage license. If are not live in partners for this period no need for this.
