Are your legal forms also applicable in the Philippines? What type of contract do we need if the we, four sisters and our husbands would like to co-own a house and lot?

Co-ownership is actually created by operation of law.If what you are asking is an evidence of co-ownership then just register the property in names of all those who contributed to the co-ownership of the subject house and lot. In this way, the whole world will be constructively notified that the four sisters and your husbands are co-owners of the property.
