how do i get medical power of attorney for a relative

Although there is a statutory form (see http://www.rilin.state.ri.us/statutes/title23/23%2D4.10/23%2D4.10%2D2.htm), the form is not exactly a model of clarity.

Given the importance of such an instrument, I respectfully suggest you find a local lawyer to help you.
