Biological parent (father)

Looking for form that spouse fills out to give op volintary custody of child in which his name is on birth certificate, and he has no objection

In order for an agreement on custody to be valid, binding, and enforcable, it must be approved by the Court and included in an Order. If you have an agreement, you should consult an experienced family law attorney to have the agreement properly drafted and then an action filed to have the Court approve it.

Attempting to find a "form" to then do it yourself will most likely result in problems down the road, which can range form small to huge.
