Changing name back to maiden after divorce

After being divorced for a decade I have decided that I want to change my last name back to my maiden name, how would I go about doing that? I have heard that it is free. Where do I get the forms? I have also heard that it is a free procedure in South Carolina, would that be true?

You must file an action in a South Carolina Family Court to have your maiden name restored. This is not free. If you know how to do it yourself, then you will still have to pay a $70.00 filing fee. I know of no place that has forms for this. The requirements are outlined in the statute. To make sure that it is done correctly, I would suggest that you contact an attorney.
