Changing custody, contempt of court

I would like to gain full custody of my children from my ex husband. I was told to settle for now, and take him back to court later by my attorney and GAL, because I had a boyfriend whom is my husband now and we had a child during my divorce. My ex commited suicide several times but it was still worse for me to have a boy friend... My ex doesn't attend hardly any of our son's doc appts for his ADHD and is frequently leaving the children for his family to care for. He allowed the children to go out of state with his grandparents w/o my knowledge. He told me he was taking them come to find out he didn't go. He also has our children around his girlfriend spending the nite at her house with and with out him. This is breaking a court order, as well as notification of trips out side of SC. Notification was givin of the trip but was told to me that he would be going as well. What can I do?? I want full custody of my children..
