child custody

do I need to file for custody of my child even though the father and I have never lived together and the child has never lived with him either. his name is not on the birth certificate, but paternity has been established

While you don't necessarily "have" to file for custody, it is a good idea to to do so clarify that you have custody and not him. You can also establish his child support obligation at that same time and have him be required to contribute toward the child's uncovered healthcare expenses, which are both a good idea.
