Custody

My sister is willing to sign over custody of her daughter to me and my husband. Where can i get the form that we need to do this?

There is not a "form" that you can use to do this. Generally, a custody agreement is negotiated and signed by both biological parents, and then the Family Court must approve the agreement in order for it to be enforceable.
