Guardianship
Can a Grandparent obtain an additional guardianship of a grandchild in addition to his parents remaining guardians also? This would be for purposes of taking child to Doctor, school conferences, etc. Things that parents would normally do, but Grandmother has to do.

Guardianship is not necessary to do the things you reference, as they can generally be accomplished via a written authorization from the parent(s).
On the other hand, it is possible for more than two people (which may or may not include parents) to share joint custody of a child.
