The court ordered (5/22/2007) that I was to receive monies from husband's retirement. Must my lawyer prepare the QDRO or may I legally approach my husband's employer to set this up?

It doesn't matter who prepares the QDRO, as long as it is prepared correctly. However, using the company's "standard" QDRO is generally not in your best interest, as it is set up to protect the company and their employee, not you. The little bit of money that you try to save can end up costing you much more in the long run. There are also companies around (like QDRO Solutions and Law Data) that can prepare the QDRO for you at a reasonable cost.
