If a mother is still married to the father of her child and she moves the 12 year old in with her and a boyfriend, then refuses the father visitation to the child. what does the father need to file for unfit parenting?

The father should consult an attorney to discuss the process for filing for custody, specified visitation, and/or an order prohibiting her from having the boyfriend present overnight when the child is there.
