You may go to http://www.state.tn.us/sos/bus_svc/forms.htm
for the forms and try to do it yourself, but you will need more than just those forms.
I would highly recommend that you hire an attorney to assist you or to do is for you.
There are lots of considerations you should discuss with a qualified professional concerning business entities. There are alternativies to an LLC that may better serve your needs that an accountant or attorney may be able to advise you on.
