I am a 30 year old male, how do I get my fathers last name if my mother has passed away

I see you are in Shelby County. I believe you will find the Probate or Circuit Court Clerks helpful with this process. They may even have some forms for you. You may wish to have an attorney do this for a fee to make sure it is done correctly, but I believe some people do this on their own. As you will be doing this for a legitimate reason, I don't think you will have any problem getting a court to grant your petition to change name. Good luck with this.
