Legal Question in Family Law in Tennessee

Guardianship

I am taking my brother for the rest of the school year and I want to know where I might be able to find the form that gives me the right of guardianship while he is in my care. I went to school and they told me I would need a form that is notarized and signed by his parents and me and my husband. They said if I did not have this I would not be able to get medical care for him if he ges hurt or anything. Can you please help me and tell me where I can find this document?

You probably do NOT need to go to court to get a formal guardianship. If you parents would sign the form included in the second of the two links, you will have authority to get medical care for your prother. Just read the links, it is all quite self-explanatory.
