Is it legal for a law firm to ask a new-
hire to fill out a form that, among
other things, asks what religion he/
she is, and what church they plan on
attending?

It is legal to ask that question, but it is probably not a good idea. For employers of 8 or more employees in Tennessee there is a prohibition against religious discrimination. This sort of question creates an environment in which that sort of claim can thrive.
