It all depends on whether the defamation of character was written (libel) or spoken (slander).
A typical letter would look something like this:
Dear Sir:
On (date) you wrote/said that I(what it was that defamed you - loathsome disease, criminal wrongdoing, etc.). That is untrue and you have defamed me. I have been damaged in the amount of $XXXXX by your wrongful act(s).
