How to dissolve a contract

I am an independent contractor for a company. The company grossly misrepresented itself on several points. Such as they said I would be the only contractor, I am not. They said my payments would come from their corporate office. They do not they come from one of there other representives. I was suppose to receive advertising materials and I did not. I was suppose to receive company standards and guidlines and I have not. I have spoken to the CEO and he is not in the least interested in my concerns. I have invested over $20,000 and they have neglected their portion of the contract. I now want to dissolve my contract and my non-compete clause. What form would I use?

There is no magic form. You should state your reasons for terminating the contract in a letter, sent certified mail, return receipt requested. If they refuse to terminate the contract, you will need an attorney to sue them for breach of contract.
