You should consult an attorney when you're making a substantial investment. This site is not intended for business people who can afford an attorney.

The answer depends on a number of factors: from whom are you purchasing the membership interest, what do the governing documents require to transfer the interest, what is the structure of the LLC. In Texas, LLCs have great flexibility but generally do not have "stock" or "shareholders" as in a corporation. You should consult with an attorney and not attempt to use a form.
