where to find notice of intent to liens forms to file being the job is done contract has been completed as well as extra work

You can go to legal zoom dot com and download a form, but I would suggest going to an attorney and having them do it for you. In Texas, if they are not done precisely then the court will deem them defective and unenforcible.
