Lien Release Form For Sub-contractors
I need a legal form, which, when properly applied, would absolve the general contractor from responsibility of payment for materials supplied by the subcontractor on a given project, when, and if, the sub-contractor fails to pay for the materials he/she supplies.

A mere form will not nullify applicable mechanic's lien statutes. Following the applicable law is the best solution.
