Subcontractor Liens

Which form would be appropriate for a sub-contractors lien against a contractor or property for non payment of labor?

First you need to send timely notice of non-payment to the owner of the real property by certified mail, with a copy to the prime contractor. Then you need to timely prepare and file a Mechanics & Materialman's Lien Affidavit in the real property records in the county where the property is located. The owner and the prime contractor also receive a copy of the affidavit by certified mail.
